As we move through 2026, Colorado Health Network (CHN) is making meaningful investments in quality improvement, ensuring our services are efficient, equitable, and centered on the needs of the communities we serve. Here’s a look at some of the key initiatives shaping how we deliver care this year.
Improving How Clients Get Connected to Services
One of the most impactful changes we’re making is building a new internal referral platform within our care management system. This system will make it faster and easier for our teams to connect clients to the right services at the right time, reducing delays and improving coordination across departments.
The platform will also allow us to track key data, such as referral volume, completion times, and service connection rates. That means we’ll have a clearer picture of where we’re doing well and where we can improve, so we can keep raising the bar for client care.

Creating Consistency Across Colorado
CHN serves clients across multiple regions of Colorado, and we know that consistency matters. A client in one part of the state should receive the same standard of service as a client anywhere else we operate.
To support that goal, we’re conducting a thorough review of regional policies covering areas such as transportation and client communications, and developing standardized approaches that can be applied across all our offices. The result will be clearer guidance for our staff and more equitable access to services for our clients, no matter where they are.
Investing in Clinical Excellence

High-quality clinical care is at the heart of what CHN does, and we’re doubling down on that commitment in 2026. Our Quality Assurance and Clinical Services teams are working together to conduct audits and chart reviews, update and standardize clinical workflows, and deliver targeted staff training on best practices and standards of care.
We’re also strengthening the connections between departments to ensure that clients experience seamless, coordinated care as they navigate our services — because no one should fall through the cracks
