Case Management
Leading a full, healthy life while managing HIV is not just a matter of finding the right medication regimen. At CHN we recognize that life is complicated and that there are many challenges that can get in the way of living your best life. Access to safe housing, complete nutrition, and reliable transportation are just a few of the daily challenges of life that can pose significant obstacles to effectively managing HIV. Our Case Management team is here to support you no matter what obstacles cross your path. Case Managers will work with you comprehensively every step of the way to coordinate access to any programs you qualify for (subject to income and residency restrictions).
Medical Case Management
Assistance for those living with HIV in finding a doctor, creating an individualized medication adherence plan, connecting with insurance options, and advocating for your specific health care needs.
Housing
Help finding short or long-term housing for those experiencing homelessness as well as assistance with rent payments, rental deposits, and more.
Nutrition
Each CHN office has a food bank to supplement access to tasty, nutritious food for clients experiencing financial hardship.
Transportation
Help is available for those who are struggling to get to medical, dental, and case management appointments.
Emergency Financial Assistance
Support may be available if you are struggling to afford essential monthly living expenses such as utility and phone bills.
