Navigating the health care system can be overwhelming. Medical Case Managers are trained to assist those living with HIV to understand how to best manage their healthcare! Our specialist team of HIV Medical Case Managers will partner with you to develop a comprehensive care plan that addresses your specific concerns.

Case Management services include a detailed needs assessment, individualized care plan, care coordination, and care monitoring.  Case Managers are here to help you every step of the way with things like finding a doctor, advocating for your specific health care concerns, and any needed community referrals designed to keep you healthy and involved in your health care process.
